Tuition Policy and Waiver Form

 

Registration fee: $35 before April 30, $45 after April 30, 2009.

Second child tuition fee 5% off.

Payment due: Payment is required in first day of class; cash or check should be made payable to Champion Learning Academy and clearly marked with the student's name. Please prepare you own snack.

Absences and late charge: a) No refund or credit for missed classes unless school cancels classes. b) Please pick up your child on time, 10 minutes for $5 late charge fee.c) School reserves the right to dismiss or suspend a childrtise in assisti for unsatisfactory behavior or physical needs for which we do not have the expeng the child.

Parents Waiver Form: I hereby allow my child to participate in the Champion Learning Academy program. I authorize the school and its staff members to take full charge of any emergency that may possibly occur. I will not hold the Champion Learning Academy, or any staff member liable in case of accidents or injuries.I agree with the Champion Learning Academy tuition and fees policy.
